Brunswick Orthotics and Prosthetics
Brunswick Orthotics and Prosthetics is a comprehensive orthotic and prosthetic provider that care for patients impacted by diabetes complications. We offer custom shoes, extra-depth shoes, custom inserts and in a few cases healing shoes if prescribed by your physician.
Your insurance provider requires specific documentation to support the medical necessity for therapeutic shoes and inserts.
In order for Brunswick to schedule your initial evaluation appointment, please gather the following documents from your Primary Care Physician or Podiatrist.
The steps are as follows:
- A prescription within the last 6 months discussing diabetic shoes, foot inserts, or shoe modifications including diagnosis. The prescription must be signed by MD or DO currently treating your diabetes.
- Physician notes from a MD or DO must discuss treatment of your diabetes and support the need for diabetic shoes as part of your care plan within the last 6 months
- If you have not seen your diabetic MD or DO within the past 6 months, you will need to schedule an appointment prior to contacting our office.
- At your initial evaluation, our clinicians will take measurements and then order prescribed devices.
- We will then schedule your fitting/delivery appointment once all your paperwork has been received and approved by insurance provider. The process can take up to approximately 2 to 3 months.
